What do you want your social media disclaimer to entail? That your opinions are yours alone and not those of your employer? That you should consult your doctor before engaging in a fitness protocol? I don't know what your goal is.

I've been asked to sign one by a company I worked for and refused, saying I'd have my attorney look it over and they never asked again.
They are all different. Theirs stated I couldn't post anything (this included my clients) via my social media page.
If your purpose is to have PT clients allow their photos to be used etc, you must state that. I'd have an attorney draw one up based on your personal needs and plans on use, especially for photos.
